The Decision Matrix Template gives you a simple but powerful way to compare alternatives using consistent criteria—so decisions don’t default to politics, volume, or whoever speaks last. This version is included with membership, including the free Jumpstart Membership, and provides a ready-to-use format for scoring options, weighting priorities, and seeing tradeoffs clearly. It’s ideal for improvement teams, leaders, and individuals who want to bring more structure to everyday decisions without overcomplicating the process. You can use this form to:

Narrow down improvement ideas

Compare solution options during problem solving

Support team alignment when opinions differ

Document why a specific choice was made

While straightforward, the Decision Matrix encourages better conversations and clearer thinking, helping teams focus on what matters most instead of getting stuck in subjective debate.
