This poster explores one of the most overlooked skills in continuous improvement: recognizing problems before they become larger issues.Designed for leaders, supervisors, team members, and continuous improvement practitioners, this poster helps build awareness of how problems are discovered, why they are often missed, and what organizations can do to create a culture that actively seeks out opportunities for improvement. Why Problems Go Unnoticed
Many problems remain hidden in plain sight. The poster examines common reasons organizations fail to recognize opportunities for improvement, including:

Complacency

Normalization of abnormal conditions

Lack of visibility

Fear of reporting issues

Firefighting cultures

Lack of ownership

It also highlights the difference between visible problems and the much larger collection of hidden issues that often exist beneath the surface.

Information Is Not the Same as Problem Identification
One of the central messages of the poster is that information alone does not identify problems. The poster demonstrates how organizations must move beyond simply seeing information and begin asking questions such as:

Why is the KPI red?

Why did production stop?

Why are we out of stock?

Why did the customer complain?

This section reinforces the idea that data becomes valuable only when people actively interpret and investigate it.

What Is a Problem?
The poster presents a simple but powerful definition:Expected Condition ≠ Actual Condition = ProblemUsing practical examples, it shows how performance gaps become identifiable problems when actual results fall short of expectations.Examples include:

Delivery performance below target

Productivity losses

Equipment abnormalities

Process failures

This provides a common language for discussing problems throughout an organization.

Key Lessons for Continuous Improvement
The poster reinforces several important concepts:

Problems rarely announce themselves

Abnormal conditions are opportunities

Finding problems is a skill that can be developed

Improvement cannot occur until problems are identified

These messages help organizations shift from reactive firefighting to proactive improvement.
<h2 id="0290f631-af4d-4eab-a61d-1fbb44656fd7" data-toc-id="0290f631-af4d-4eab-a61d-1fbb44656fd7" style="text-align: left; This poster helps teams develop the awareness, observation skills, and mindset needed to identify opportunities for improvement before they become larger operational, quality, safety, or customer issues.
